The generated file was being indexed

It was enough. The indexer had no ignore rule for it, so every keystroke re-tokenised 40MB.

Adding it to the ignore file took the open from six seconds to instant. What made this hard to find is that the editor reported no error at any point; it was simply doing an enormous amount of correct work.
