A postmortem with no action items

We wrote one. Just what happened, in order, with the times.

It was the most useful one we have written, because the action items in the others were mostly a way of ending the conversation. Reading the sequence, three people independently noticed the same gap, and the fix that came out of it was not on anyone's list.
